registration New Patient Registration - please note that our practice list is currently closed.

The following will only apply when we re-open our practice list.   If you wish to register with the practice and reside within our practice boundary area please call at the surgery to discuss registration (a map of our practice area is available at reception). You will need to bring your medical card with you. If you are unable to locate your card, we can provide you with an application form or alternatively you can print a form (please see the Registration Form page) to complete and bring to the surgery.

Should you be unable to attend in person, someone can make the registration on you behalf, with your permission. You will register with the practice, not with a particular doctor, but you will be asked to specify your preferred doctor when registering. You may request to see a doctor of choice at the practice, however, there may be delay if you choose to wait to see a particular doctor.

New Patient Medicals

All newly registered patients aged five and over are invited to attend the surgery for a health check when joining our practice. This includes a brief look at your medical history with details of medication and advice about health including weight, diet, exercise and blood pressure. Please bring a urine sample with you to this appointment.
