 WELCOME TO NEW ROAD SURGERY

The practice provides a full range of GP services, this leaflet describes these services. Please read this leaflet, and keep it for future reference.

Reception telephone service available from 08:30am. Please telephone after 10.30am to book routine appointments and during the afternoon to request results etc. If you wish to speak to a doctor or nurse by telephone this can be arranged via reception.

 HOME VISITS

Please note that the surgery is the best equipped place for consultations. If a visit is absolutely necessary, please make the request before 11am wherever possible.

EMERGENCY CALLS

When the surgery is closed Dial 111

0121 453 3584 will remind you to call 111 (answer machine does not take messages) .

Telephone advice, an appointment with a doctor, or a home visit will be offered. This service is intended for emergencies, or problems that cannot wait until the next morning surgery.

 IN CASE OF LIFE-THREATENING EMERGENCIES DIAL 999

APPOINTMENTS Routine appointments with a doctor are on average 10 minutes in duration. If you feel that you have a lot to discuss, please ask for a longer appointment to avoid delay to other patients.

If you no longer require, or are unable to keep your appointment, please let us know so that we can use it for someone else.

REPEAT PRESCRIPTIONS

These are available for some chronic or recurring conditions.

Please make requests in the following way:

  • handing in a request slip (attached to your prescription)
  • completing a request form (available from reception)
  • online - register for online services please attend the surgery with photo ID & address confirmation (i.e. passport or bank statement) and register for this service
  • *by email —the address is RBCCG.oohnewroad@nhs.net

*Please note that confidentiality and security of emails can not be guaranteed whilst in transit and should contain the minimum amount of patient identifiable or confidential information. We will print then delete your email on receipt but it will be stored on your email providers server .

Your prescription will be ready to collect 2 working days after the request is submitted.

Telephone requests will not be taken, other than for housebound patients.

NEW PATIENTS

If you wish to register with the practice please call at the surgery to discuss registration. You will need to bring your medical card with you; if you are unable to locate your card, we can provide you with an application form instead. If you are unable to attend in person, someone can make the registration on your behalf, with your permission. You will register with the practice, not with a particular doctor, but you will be asked to specify your preferred doctor when registering. You may request to see a doctor of your choice at the practice, however, there may be delay if you choose to wait to see a particular doctor.

All new patients are invited to make an appointment to have a health check with our practice nurses.

THE COMMUNITY TEAM

DISTRICT NURSES

District Nurses are available to see you in your home if you are housebound. They give advice and care to the acutely and chronically ill.

HEALTH VISITORS

Health Visitors provide help and advice particularly to families with children under the age of 5 years old. They are also available for advice on matters concerning general health and nutrition.

OTHER INFORMATION

COMPLAINTS - We try to offer our best service at all times. If however, you are unhappy with any aspect of your care, please either write or ask to speak to the practice manager. Information about how to complain is available at reception and on the practice website.

SUGGESTIONS - We welcome your suggestions, and a suggestion box is situated in the reception area.

PERSONAL DETAILS - Please inform us of any change of name, address or telephone number. If you have an ex-directory telephone number, please let us know.

VIOLENT OR ABUSIVE BEHAVIOUR - This practice supports the Governments NHS zero tolerance zone campaign. Violent patients will be reported to the police and struck off the practice list. We ask you to treat your GP and practice staff properly - without violence or abuse.

CONFIDENTIALITY STATEMENT - This is a completely confidential service. Even if you are under 16, doctors and nurses still have to keep anything you tell them private, just as they would for an adult. We will not discuss any information about you with anyone else, unless you ask us to, except in very exceptional circumstances - if you, or another’s safety is at risk. In these circumstances, we will keep you fully informed.

CARERS - if you are a carer please let us know so that we can make a note of this on your medical record.

 OTHER SERVICES AVAILABLE

CERVICAL SCREENING - is carried out in accordance with guidance from the NHS Cervical Screening Programme.

CONTRACEPTIVE SCREENING - appointments are available during normal surgery times. Doctors offer a range of contraceptive services. The nurses are happy to offer advice on any form of contraceptive and will perform relevant health checks when necessary.

Advice on all contraception (including post coital) is confidential to all.

 

ADULT VACCINATIONS AND IMMUNISATIONS - will be administered in accordance with current guidelines. For travel vaccinations please aim to commence vaccines three months before your travel date wherever possible. Please ask at reception for a travel questionnaire which you will need to complete prior to your appointment with the nurse.

HEALTH CHECKS - Patients who have not been seen by a GP in the past 3 years can request a consultation for a general health check. (1 year for patients aged 75+)
