If you would like to register at New Road Surgery please complete and sign the attached form. The form needs to be posted to the surgery either by hand or via royal mail. Please note that we can only register you at New Road Surgery if you reside within our boundary area.
IMPORTANT NOTE: From 5/1/2015 practices may choose to register patients who reside outside their boundary area. This arrangement is optional for practices. Please note New Road Surgery will continue to only register patients who reside within it's existing practice boundary area.
Practice Boundary Change with effect from 1/7/2019 we will no longer accept new registration requests from people residing in the following areas: Catshill, Lickey End & Blackwell. This change has been agreed with NHS England and is as a result of an increased number of patients now registered at the practice.

Should you be unable to attend in person, someone can make the registration on you behalf, with your permission. You will register with the practice, not with a particular doctor, but you will be asked to specify your preferred doctor when registering. You may request to see a doctor of choice at the practice, however, there may be delay if you choose to wait to see a particular doctor.
Newly registered patients aged five and over are invited to attend the surgery for a health check when joining our practice. This includes a brief look at your medical history with details of medication and advice about health including weight, diet, exercise and blood pressure. Please book your appointment at reception. Please bring a urine sample with you to this appointment.
The doctors are prepared to see any of your family and friends who need a doctor when they are staying with you. Similarly, when you are away from home in the UK you can visit any local doctor as a temporary resident, without having to register as a permanent patient at that surgery.
Sign language interpreters can be booked for face to face consultations at the surgery. Please let reception staff know if you require an interpreter and they will arrange this for you. Please note there may be a short wait for this service so please let us have as much notice as possible so that we can find a suitable appointment slot that both you and the interpreter can attend. Please note there is no charge for this service.
The practice can arrange translation services for patients. Please ask at reception if you need a translator.
